The Labret stud is worn in the labret piercing (between the chin and lip). It should not be worn in the lip piercing as the back can irritate the gums over the teeth and wear them away. This can be permanent. Worn in a correctly placed labret piercing it can still cause some 'bedding' but will usually settle after a little time. Some people cannot have a labret piercing as they do not have any space below the gum line.
